Streamline Your Invention Disclosure & Income Distribution

UCSF offers confidential disclosure and income distribution forms to support innovation. Use the online Inventor Portal to disclose inventions, including devices, research tools, software, and data packages. After submission, you'll receive a case number and contact from a Business Development expert to explore commercialization opportunities. Income distribution forms are available for multiple inventors or for allocating software/app income differently than default policies.

1. Disclosure Form

Invention Disclosure is now online!

UCSF faculty, staff, and students can submit an invention disclosure online using the new Inventor Portal.

If you're a returning inventor or ready to disclose now, go directly to the new portal, where you'll be prompted to log in with your MyAccess credentials or create a new account.

Use this form for all disclosures, including diagnostics, therapeutics, medical and mechanical devices, methods of use, manufacturing processes, laboratory instruments, and equipment; research tools such as a cell line, transgenic model, expression vector, or research-use-only antibody (Tangible Research Properties); software, mobile apps, or other copyrightable material such as surveys or questions; and data packages such as preclinical, in vitro or in vivo data, clinical data, or regulatory data.

What Happens After You Submit Your Disclosure Form?

After submitting a disclosure form, you will receive a notification email with the case number, name, and contact information for the Business Development Manager assigned to your disclosure. They will contact the inventors to discuss the technology's details and potential commercialization opportunities. The information you submit will be kept confidential. Note that an invention disclosure form by itself does not constitute a patent or patent application; it is solely a record of your invention for UCSF's internal use.

2.1 Invention Income Distribution Forms

Use this form if you have two or more inventors and all the inventors agree on an income distribution different from that described in the UC Patent Policy. This form must be signed by all inventors and sent to Innovation Ventures via email.

2.2 Software and Apps Income Distribution Forms
